MULTIPLE SCLEROSIS-التليف العصبي المتعدد
- هو مرض يصيب غلاف الاعصاب بالجهاز العصبي المركزي .
- يصيب شخص واحد من كل الفين شخص في بريطانيا سنويا .
يعتقد ان المرض عبارة عن نشوء اجسام مضادة لغلاف الاعصاب فيحدث المرض .
- لم يعرف له اية علاقة جرثومية .
- تقل الاصابات في المناطق المعتدلة المناخ وتزداد في المناطق الشمالية والجنوبية ؟؟
- بعض الجينات الوراثية تزيد احتمالية الاصابة .
- تصيب بعض الاقارب وهدا يؤكد وجود العامل الوراثي .
- بعض الدراسات تؤكد وجود عامل الاجسام المضادة الداتية .
PATHOLOGY :
- يكون موضع الاصابة على هيئة بها تدمير شامل لغلاف العصب مع المحافظة على المحور الاساسي للاعصاب .
- يكون لون هدة المواضع مائل الي الوردي لوجود الشعيرات الدموية .
- يكون الموضع مزدحم بكريات الدم البيضاء .
- في اوقات متأخرة يبداء تليف الاعصاب وتنكمش ويتحول لونها الي الرمادي .
- تكون الاصابة متوزعه على المادة البيضاء بالدماغ والحبل الشوكي وحول التجويفات الدماغية والاعصاب البصرية .
الاعراض :
- وهي تختلف من حالة الى اخرى .
- تكون الاعراض على شكل نوبات ثم فترة راحة .
- تعتمد الاعراض على موضع الاصابة بالدماغ.
- غالبا ما يكون عمر المريض بين20-40 سنة .
- تصاب النساء اكثر من الرجال .
- تفشل احدى الاطراف السفلى.
- قد تصاب احدى اعصاب البصر retrobulbar neuritis .
- ازدواجية البصر diplopia .
- دوخان وفقد الاتزان – نوبات صرع – صعوبة بالكلام .
- شلل نصفي – شلل الوجة .
- في خلال 1-3 اشهر تختفي هدة الاعراض .
التشخيص :
- يعتمد التشخيص عاى وقوع الاعراض في اوقات مختلفة ومواقع مختلفة من الجهاز العصبي المركزي .
D.Dx. الامراض المشابهه : يجب التأكد من عدم وجودها قبل وضع تشخيص MS .
1-نقص فيتامين B12 -والدي يسبب تليف جزء من النخاع االشوكي .
2- المرحلة العصبية للزهري .
3- الفثق الغضروفي بالعمود الفقري .
العلاج :
- لايوجد علاج شافي الي الان .
- يعطى المريض الكورتيزون

Modern medicine has failed to find an effective treatment for the overall condition although various drugs give short-term relief of different symptoms. Valium or similar tranquilizers are used to treat muscle spasms but can have the associated side effect of addiction, and doses often have to be increased sharply over time. MS patients who use Cannabis report a soothing of the painful muscle spasms and improved muscle coordination. Some are able to walk unaided when they were previously unable to do so. It also helps blurred vision, tremors, loss of bladder control, insomnia and depression.
During symptomatic attacks, patients may be hospitalized and are typically given high doses of intravenous corticosteroids, such as methylprednisolone, to end the attack sooner and leave fewer lasting deficits. For relapsing-remitting MS, there are 5 FDA approved drugs available for treatment. Three are interferons: Interferon beta-1a (Avonex and Rebif) or beta-1b (Betaseron [in Europe Betaferon]). A fourth medication is glatiramer acetate (Copaxone), a mixture of polypeptides which may protect important myelin proteins by substituting itself as the target of immune system attack. The final medication, mitoxantrone is effective but is limited by cardiac toxicity. All five medications have been proven to be modestly effective at decreasing the number of attacks and slowing progression to disability. All of these therapies are expensive and require frequent injections, with Avonex requiring weekly injections and Copaxone daily injections. All of the interferons can lose effectiveness after continued use, with Avonex being the least likely and Betaseron the most likely. This is the result of neutralizing antibodies against the interferons. The interferons all require laboratory monitoring of blood tests. Even with appropriate use of medication, most patients with relapsing-remitting MS still suffer from some attacks and subsequent disability.
